What Disability-Related Changes Do You Want to See in Our State?

  • This survey is your chance to tell FOUR statewide disability agencies what you need. We will ask questions that help us know:
    • What’s working well for people with disabilities in Tennessee?
    • What’s not working well?
    • What support gaps need to be filled?
  • The survey will take about 15 minutes. It is available in both English and Spanish.
  • Our organizations will use this information to decide what change to work on for the next 5 years.

Why Do We Need This Information?

Every person with a disability, as well as their families, should have the support they need to lead a fulfilling life in their community.

The Tennessee Developmental Disabilities (TNDD) Network has partnered to carry out a statewide community needs assessment. This survey will help our organizations understand the needs of the disability community across our state. Each partner is preparing to write a new five-year workplan. These plans outline the future goals and priority areas of each organization. Before these plans can be developed, we need input from Tennesseans.

The work of the TNDD Network is guided by the voices of Tennesseans with disabilities and their family members. This survey is an important way for us to hear from you. Without this input, we would not have the information we need to guide our work and would only be guessing where to put our resources. That is why we are asking you to take 15 minutes to complete this survey. If you need, you can save your progress and come back to finish your answers later.

We know your time is valuable! One of the reasons for this partnership is to reduce the number of surveys we ask you to complete. We created one statewide needs assessment, rather than asking you to complete a survey from each organization, like in years past.
